Requirements for a New Salon (Beauty, Nail or Esthetic)
The first step in licensing a salon in the Commonwealth of Kentucky is to complete the application for a beauty salon license. The application requires three (3) signatures:
The owner of an establishment is not required to be licensed, but in all instances, the manager of a beauty salon must be a licensed cosmetologist; the manager of a nail salon must be a licensed cosmetologist or nail technician; and, the manager of an esthetic salon must be a licensed cosmetologist or esthetician.
Once the zoning and plumbing portions of the application has been completed, you should contact the cosmetology salon inspector to schedule an inspection time. This is the last person to sign the application and the establishment must be ready for opening when the inspector arrives. See New Salon Application Checklist.
The application must be submitted to the Board office with the appropriate fee. Please allow for sufficient processing time as the license must be posted in the salon prior to opening. You may bring the completed application to the board office and the license will be issued to you at that time.

Change of Owner/Manager |

If a salon changes owner(s) or manager(s) at anytime during the licensing year, a new salon license must be purchased (KRS 317A.050(4)(a) and (b); KRS 317B.025(3)).
Please complete the Application for Change of Owner/Manager and submit the completed application to the board office along with the appropriate fee.
NOTE: If there are any changes in the plumbing (i.e. replacing old shampoo bowls, adding shampoo bowls, pedicure stations, etc.) a new salon application must be completed with the appropriate signatures.

If your salon is changing locations and the new location has not been licensed as a salon within the last six (6) months, you must complete a new salon application.
If the new location is currently licensed as a salon or was operating and licensed as a salon within the last six months, and there has been no plumbing changes, you can complete the Application for Change of Owner/Manager.

Independent Contractor Salon License |

All licensees who rent or lease space in a licensed salon must obtain an independent contractor license by completing the Application for Independent Contractor License.
The application must be submitted to the board office along with the appropriate fee.
If you are an independent contractor in more than one salon, you must have an independent contractor license for each location.
Anytime that you change salons or locations, you must purchase a new independent contractor license. If the salon where you rent space moves to a new location, you must also purchase a new independent contractor license.
